Please have your insurance information ready when you call to make an appointment. You will be responsible for any co-pays and non-covered items at the time of service. We will do our best to help answer any questions you may have about your medical or vision insurance before your exam, but if you have any additional inquiries, you may want to contact your insurance company yourself or the human resources department at your place of employment.
